Our team specializes in health system innovation, transformation, and optimization. We have extensive experience developing evidence-informed frameworks, evaluation models, logic models, and key performance indicators that guide decision-making and support sustainable improvement. Jeff

Co-founder

Brockmann

Jeff Brockmann is a senior healthcare executive with over 14 years of executive leadership experience in publicly funded healthcare environments, Jeff has led complex, multi stakeholder initiatives requiring strategic planning, governance alignment, performance oversight, and interdisciplinary integration. As Executive Director of a community-based primary care organization operating within a Primary Care Network and provincial health system context, Jeff has been directly accountable for organizational strategy, operational performance, financial stewardship, regulatory compliance, and system integration. He led the full implementation of the Patient Medical Home model, redesigning governance structures, care delivery processes, team-based care models, and performance reporting frameworks to align with provincial priorities and accountability requirements. Jeff’s leadership experience includes facilitating interdisciplinary planning sessions, aligning physicians and clinical teams around shared objectives, developing evidence-informed strategic roadmaps, embedding quality improvement methodologies, and strengthening reporting and evaluation systems. He has worked closely with Primary Care Network leadership, Alberta Health, AHS partners, and community stakeholders to advance coordinated, sustainable system improvements. Jeff brings practical experience in translating strategic direction into executable implementation plans, identifying and mitigating operational and governance risks, and sustaining performance within fixed public funding environments. His approach emphasizes structured project oversight, transparent communication, and measurable outcomes to ensure initiatives are both strategically sound and operationally achievable. He holds a Bachelor of Arts in Business Management and has contributed to peer-reviewed research on team-based primary care.

Summer

Co-founder

Wesolosky

Summer Wesolosky is a senior health system leader and certified project and change management professional with more than 15 years of experience leading large-scale, province-wide initiatives within Alberta’s public health system. Recently serving as Senior Program Manager with Alberta Health Services, Summer has played a pivotal role in advancing collaborative, patient- and family-centred care across diverse acute and community settings. Throughout her career, Summer has led complex transformation initiatives from concept through implementation and evaluation, with a proven ability to navigate evolving systems, align stakeholders, and mitigate risk. She was instrumental in provincial implementation engaging units to improve acute care flow, strengthen discharge planning, and enhance both patient and provider experience. Her work contributed directly to supporting key AHS strategic objectives, including workforce resilience, coordinated care delivery, and improved patient outcomes. Summer brings deep expertise in implementation science, performance measurement, and workforce optimization. She has led qualitative and quantitative evaluation efforts reported to Alberta Health and the Minister of Health, with a particular focus on addressing workforce shortages in rural, remote, and Indigenous communities. Known for her collaborative leadership style, she excels at fostering cultural change, coaching leaders and frontline teams, and building sustainable frameworks that improve efficiency, retention, and quality of care. In addition to her professional leadership, Summer is a committed volunteer and past chair of Emerging Health Leaders – Edmonton Node, where she has supported leadership development and community engagement within the health sector. She holds a Bachelor of Science from the University of Alberta and multiple certifications in project management, change management, and Lean methodology.

Jennifer

Brockmann

Jennifer Brockmann is a strategic health system leader and project manager with over 18 years of experience leading complex, publicly funded initiatives across healthcare and related sectors. She is completing a Master of Counselling Psychology (expected May 2026), further strengthening her expertise in systems-informed clinical and organizational practice. She also holds a Bachelor of Social Work and has been a Registered Social Worker (RSW) for 19 years. Her work spans primary care, acute care, continuing care, addictions, and mental health, with a particular focus on rural and community-based service delivery. Jennifer brings deep expertise in project and change management, facilitation, coaching, implementation science, and quality improvement. She is PROSCI-certified in change management and has training in Lean and Six Sigma methodologies, knowledge translation, and team-based care. Her practice is grounded in translating evidence, policy, and system priorities into practical, sustainable solutions that improve access, care coordination, workforce resilience, and patient and family experience. Throughout her career with Alberta Health Services, Chronic Disease Management, and Primary Care, Jennifer has led province-wide transformation initiatives, including the design and implementation of collaborative care frameworks, primary care access and attachment improvements, patient medical home implementation, and chronic disease management programs. Her leadership has supported measurable improvements in care coordination, discharge planning, screening, access to services, and interdisciplinary team functioning across urban, rural, and remote settings. Jennifer is recognized for her ability to work across sectors and disciplines, engage diverse stakeholders, and guide teams through complex change in high-accountability environments. She is known for fostering cultures of collaboration, continuous improvement, and patient-centred care while ensuring alignment with governance, regulatory, and reporting requirements.
